Easy read guides to health choices and questions to ask
Adapted in consultation with experts by experience into easy read from the NICE shared decision making guidance, the main emphasis is making sure people understand they have a choice about their health and care. The questions are: What are my choices? What is good and bad about my choices? Can I get support to help me make my choices?
Why read this NHS England works with people with a learning disability. People should get information in a way that is easy for them to understand. The Accessible Information Standard tells the NHS to do this.

Easy read information is presented in a certain way to help people understand information more easily. While there is no single ‘right way’ to do this, this guide will help you to understand the basics of producing easy read resources. This guide has been produced by Your Health Matters, part of the Leeds and York Partnership Foundation Trust.
There are lots of ways to get help. 1. You can contact your doctors practice (GP) to get an appointment. Check how you can do this with your doctors practice. Your appointment may be on the phone, or a video call or in person. 2. You can go to your local pharmacy (chemist) for advice.
